Advance care planning involves planning for your health.

It enables you to make some decisions now about the health care you would or would not like to receive if you were to become seriously ill and unable to communicate your preferences or make treatment decisions.

Facilitated by Kelly Arthurs, a local Nurse Practitioner, the workshop will cover:

  • What is advance care planning and why it is so important to discuss.
  • What are the most important aspects to consider with advance care planning.
  • Q&A with a local special guest.
  • Opportunity to reflect, have a conversation, and commence your own advance care planning journey including what to do next.

Morning tea provided.

This is a Dying to Know Day event brought to you by Sydney North Health Network and NSW Health, Northern Sydney Local Health District.
